Data Consent Form

I hereby give my consent to Bamberry Consulting to process the following information: personal data • name • date of birth • contact details, including telephone number, email address and postal address • experience, training and qualifications • CV and educational history, qualifications and skills • national insurance number and tax and payroll information • passport, visa and other right to work identity information • bank details • next of kin and family details • contact details of referees • Information contained in references and pre-employment checks from third parties sensitive personal data • disability/health condition relevant to the role • criminal conviction. I consent to Bamberry Consulting processing the above personal data for the following purposes: • to provide me with work-finding services. • to process with or transfer my personal data to their clients in order to provide me with work-finding services. • to process my data on a Bamberry internal database in order to provide me with work-finding services. I also consent to Bamberry Consulting processing my personal data with their clients for the purposes of internal audits and investigations carried out to ensure that Bamberry Consulting are complying with all relevant laws and obligations. I also consent to Bamberry Consulting processing my personal data with third parties including The REC for the purposes of internal audits and investigations carried out to ensure that the Bamberry Consulting is complying with all relevant laws and obligations. The consent I give to Bamberry Consulting will last for 36 months. I am aware that I have the right to withdraw my consent at any time by informing Bamberry Consulting that I wish to do so.
